Form I-134 is made so that you can document that you provided support as required by the Foreign Account Tax Compliance Act (FATWA). If you are required to file a claim for refund because of a tax debt due to a failure to file an income tax return, you should indicate that you have filed Form I-135, Statement of Bankruptcy, when you submit these attachments.
